LINFOMA DE CÉLULAS B GRANDES DIFUSO CON DOBLE EXPRESIÓN E INFILTRACIÓN ÓSEA: REPORTE DE CASO

El linfoma de células B grandes difuso (LCBGD) puede infiltrar el sistema óseo de forma secundaria, presentándose con dolor inespecífico que dificulta su diagnóstico. Se describe el caso de un paciente masculino de 48 años con dolor persistente en la rodilla izquierda durante tres años, inicialmente tratado como hiperuricemia sin mejoría. Resonancia magnética evidenció una lesión ósea en la tibia con compromiso de tejidos blandos y adenopatías inguinales. La tomografía por emisión de positrones con tomografía computarizada (PET-CT) confirmó infiltración tumoral ósea multifocal. La biopsia demostró LCBGD con doble expresión de C-MYC y BCL-2 y un Ki-67 del 90 %. Se inició tratamiento con R-CHOP (rituximab, ciclofosfamida, doxorrubicina, vincristina, prednisona), intensificado a R-DA-EPOCH. El paciente presentó buena tolerancia y mejoría clínica.
