Primary hypoparathyroidism Exacerbated by dengue with seizures: a case report

Primary hypoparathyroidism is a rare endocrine disorder characterized by low parathyroid hormone (PTH) and serum calcium levels. We report a 25-year-old male presenting with generalized seizures. Initial studies revealed severe hypocalcemia (5.3 mg/dL), low PTH (5.7 pg/mL), thrombocytopenia (68,000/μL), and elevated liver enzymes, associated with confirmed dengue (NS1 antigen positive). Brain computed tomography and magnetic resonance imaging showed no calcifications typical of hypoparathyroidism. The electroencephalogram revealed cortical irritative activity. Treatment included intravenous calcium gluconate, teriparatide, and dengue management. This case highlights how dengue can exacerbate underlying endocrine disorders. The absence of basal ganglia calcifications emphasizes variations in the clinical presentation of hypoparathyroidism triggered by acute infections.
