Deep infiltrative endometriosis is a chronic inflammatory disease associated with pelvic pain and infertility, often diagnosed for several years. We present the case of a 38-year-old patient with disease recurrence after multiple medical and surgical treatments over 18 years. Specialized ultrasound mapping identified multifocal involvement and allowed for the planning of an interdisciplinary surgical approach based on the #ENZIAN classification. After 24 months of follow-up, the patient showed sustained clinical improvement and no significant recurrence.
