Giant Phyllodes tumor in pregnancy

Phyllodes tumor (PT) is a rare fibroepithelial neoplasm that accounts for approximately
1% of all breast tumors and is histologically classified as benign, borderline, or malignant
according to stromal cellularity, cellular atypia, and mitotic activity. Clinically, it usually
presents as a well-circumscribed breast mass with a firm consistency and variable growth
rate. The diagnostic approach includes imaging studies and histological confirmation
through core needle biopsy. The treatment of choice is complete surgical excision with
adequate margins, although mastectomy may be required in cases of large or malignant
tumors. We report the case of a 22-year-old patient with a giant malignant phyllodes tumor of the right breast associated with a 15-week pregnancy by last menstrual period, who
underwent total mastectomy with partial resection of the pectoralis major muscle and
placement of a vacuum-assisted closure (VAC) system.
