The lengthening and correction of bone deformities has undergone multiple modifications;
the decision to treat is based on the massive bone defect secondary to a post-traumatic bone
infection. We present the case of a 7-year-old female patient, with a multifragmentary
fracture of the middle third of the left femur. It is planned at first time removal of
osteosynthesis material, taking cultures, macerates, taking biopsy, placement of cement
spacer with antibiotic. In a second time, the first masquelet time is performed and an
autologous bone graft of iliac crest and fixative for bone lengthening, membrane transport
and graft compression are placed. In the third period, removal of the monorail and
osteosynthesis of the left femur with plaque is planned, obtaining satisfactory results and
adequate bone consolidation.
