Colonic lipomas are rare benign tumors first described in 1757. They account for 0.035% to 4% of colonic tumors, primarily affecting women aged 40–50 years. Case Report: We present a case of a 70-year-old male with no relevant medical history, who consulted for diffuse abdominal pain and changes in bowel habits. Imaging studies identified a subepithelial lesion in the ileocecal valve, which was confirmed as a lipoma through biopsy and histopathological analysis. Discussion: This case highlights the importance of combining imaging studies and endoscopy with biopsies for the differential diagnosis of subepithelial lesions, such as lipomas, GISTs, and leiomyomas. Conclusions: Ileocolic valve lipomas are rare subepithelial lesions requiring an accurate diagnostic approach to differentiate them from other submucosal tumors.
